If anything helps destroy earphone quickly, it's the gym. Unfortunately, most sets simply aren't tough enough for the rigors of a workout, which is why "sport" models are increasingly popular. The MEElectronics Sport-Fi S6 ($49.99 list) offers plenty of interesting features for the exercise-minded, including an armband, a sweat-resistant design, and a memory wire. Unfortunately, sound quality takes a backseat here. The Sport-Fi S6 is a great grab-and-go package for runners and other fitness buffs, but it's only good enough for casual listeners.

Design and Fit
The Sport-Fi S6 comes with a neoprene sport armband for your phone or MP3 player, two shirt clips for the wire, a zippered cloth carrying case, and six pairs of silicone ear tips in various shapes and sizes. This is great for the gym fanatic, as you probably don't need anything else for your exercise routine.?You also get a three-foot extension cable, just in case you need it for carrying your phone or MP3 player further away, say, in a backpack.

MEElectronics paid attention to style with the Sport-Fi S6, and it pays off: The racy, red and black color scheme looks sharp, and all of the accessories match nicely. Each Sport-Fi S6 earpiece features rubber tips that sit at roughly a 45-degree angle. The noise-isolating design doesn't actually cancel noise, but it certainly helps mitigate it in louder environments. I had no problem using the default ear tips, though it's nice that you get a choice of one, two, and three-ring designs as well as a selection of sizes. Getting a proper in-canal seal is a little tricky, though, and you'll need one for proper bass extension and punch.

The memory wire isn't removable, so if you break it, you can say goodbye to the earphones. But the cable lives up to its namesake, as it has some (but not a complete) ability to retain its shape around your ears over a period of time?and is also helpful for positioning it just the way you want it while working out.?If you don't need the sport accessories or memory wire design, consider stepping down to the pedestrian-looking MEElectronics M6 ($29.99), which offers the same sound quality.

Performance and Conclusions
I tested the Sport-Fi S6 with a Google Nexus 4 smartphone. On the sound quality front?arguably not the Sport-Fi S6's main mission, though clearly still important?things go downhill a bit. The Sport-Fi S6 delivers boosted, slightly harsh lower and mid treble, and there's also too much boost in the upper bass region, which leads to a boomy, unrefined sound. Down below, there's some actual mid-bass response, so you do get a satisfying punch out of kick drums.

In my tests, Green Day's "American Idiot" was tough to hear, thanks to excessive harshness during the power chord choruses. The drummer's hi-hats in System of a Down's "Toxicity" sounded too crisp and almost electronic, even though they're real cymbals. Queens of the Stone Age's "No One Knows" lost some of its punch and clarity, but was still easily listenable. In back-to-back comparisons, Apple EarPods ?sounded smoother and more natural than the Sport-Fi S6 on the same material, with a less fatiguing sound. Impressively, the EarPods also deliver some actual bass punch, even though it's an easier-to-fit, earbud-style design that doesn't create a seal.

Overall, the MEElectronics Sport-Fi S6 is a decent buy if you need the accessories that come with it, and it certainly does the job as a proper sport set of earphones. But Apple EarPods cost less and sound more natural. While they're not true sport earbuds that are resistant to sweat, their plastic design and on-ear shape lend themselves well to use while working out. If you have a little more cash and are after pure sound quality, the Editors' Choice-winning AKG K 350 ?is a killer value with full-range, transparent, satisfying sound and more-detailed response, although it lacks the sport armband and other accessories you get with the Sport-Fi S6.

Six Lessons from a Former Mutual Fund Investor

I have been studying investing since my undergraduate college days where I studied Finance. ?Ironically, it wasn?t for many years after college that I really started questioning the common advice being offered by mainstream financial planners.

When I did, however, I realized how wrong that common advice was and totally changed my approach. ?Since then, I have been involved in the financial services industry, now concentrating on structuring Equity Indexed Universal Life Insurance policies that creates tax-free income for my clients -though I will not be limiting my posts to EIULs (boy would those be snoozers!) Instead, I will talk about all things surrounding investing and the economy. ?I have been a long-time fan of real estate investing and believe that owning an EIUL is a perfect complement to real estate.

This first post I want to briefly talk about my voyage from the standard, ? mutual funds inside a 401k/IRA wrapper,? strategy for retirement income to where I am now. I?m sure many of the readers have had similar voyages. ?There are some lessons to be learned from our earlier mistakes.

Signs of Trouble

In the late 1990s I started to get a funny feeling about my retirement funds which were invested in mutual funds inside an 401K and an IRA. ?There was so much emphasis on ?the number? or the numeric goal you hoped to reach by retirement time that it seemed to me to simplify the process too much. ?At the same time people were assuming very high average rates of return from their mutual funds, some even over 20%. ?That is when I started to really look at historic stock market returns and immediately noted several huge issues. ?The first was ?the actual sequence of returns from the stock market was highly variable creating what is called ?sequence of return risk.?

It became apparent that a large market swoon within 5 years before or after retirement was not only likely, but almost guaranteed. ?The only way to avoid this was to move out of the stock market sometime earlier which of course kept your overall returns low at a critical time. ?The other issue was that you never could really get a handle on how long this money was needed to last for? (or put uglier: when you were going to die.)

It became apparent that the cycles in the stock market were very long and unpredictable so it was entirely possible that you would spend a large portion of your investing life in bad stock times. Finally, it wasn?t entirely clear how best to turn your mutual fund wealth into retirement income.

To put this in technical terms there were three risks unaccounted for by mutual funds; sequence of return risk, economic risk, and length of life risk. ?It was easy to see how any of these risks could create havoc even with a carefully planned retirement strategy that depended upon mutual funds no matter how they were balanced.

It also became apparent that there were opposing camps of advice that seemed to be more interested in fighting each other over client dollars than to figure out what were the best retirement strategies for folks. ?The stock salesmen and the insurance salesmen spent much of their time demonstrating their opposition?s approach was dangerous instead of actually looking at the results of their advice that was streaming in.

By the way, the results of their advice was, and still is very ugly.

The other idea I found somewhat vexing was the idea of diversification. ?There were many good to great investors that simply did not believe in the need for massive diversification of stocks and even the original work done on diversification was of diversified asset classes not diversification within an asset class.

Six Fundamental Ideas to Keep In Mind

So that is where I found myself with all these ideas rolling around in my head and no strategy in place to account for them.

These? ideas are important and should be kept in mind for all investors:

1. It?s not ?the number? that is important, but the income generated by the investments;
2. Variance is an enemy of retirement income seekers;
3. Deferring income taxes is a losers strategy because no one can predict what the income tax rates will be in the future and [here is an obvious one] our goal should be to have as much income as possible, pushing us up to higher income tax brackets in the future if we defer the tax;
4. One really good investment is much better than 10 mediocre ones;
5. Time is always a question mark with investing, so investments should be able to work together to account for the unknowables around how long we live, how long we can actively manage our investments, how long we have to produce money for investments, etc.; and
6. Whenever the government and a large influential corporate cabal get together to make you a deal that seems great remember they are not doing this for your benefit but for theirs!

The Path Less Traveled

My search for answers found me focusing on three areas:

• Real Estate;
• Insurance products;
• Growth stocks (including dividend producing stocks.)

It took me a while to get out of my 401k with mutual funds but after around 8 years I accomplished it. ?This is all described in my blog, which I started in 2008. ?During my journey I made a decision to get my insurance license and sell Equity Indexed Universal Life Insurance because I found the product so useful and the average life insurance agent so ignorant on how to use it.

It has been a strange journey to having a niche life insurance business structuring policies for maximum cash value build-up, but it has been fun.

I hope to share much of what I have learned with BiggerPockets. ?Along the way if you have any questions or suggestions for posts please let me know, I can use all the help I can get

Obama takes ?fiscal cliff? plan on the road: Weekend in politics

President Barack Obama takes his plan for avoiding the "fiscal cliff" on the road Friday with a tour and remarks at The Rodon Group Manufacturing Facility in Hatfield, Pa.

The White House is looking to drum up public support with the campaign-style event for a plan that includes $1.6 trillion in new tax revenues plus some $400 billion in savings in entitlement programs. It also folds in relief for Americans hit by the home foreclosure crisis and an extension of the payroll tax holiday and unemployment assistance. There's also a $50 billion jobs plan that's mostly infrastructure spending.

The White House definitely will be looking for a better reception Friday than the plan got Thursday when Treasury Secretary Timothy Geithner presented it to Republican House Speaker John Boehner. Congressional Republicans rejected it as nothing more than the same plan Obama has been pushing since last year, including tax increases on the richest Americans, Yahoo News White House Correspondent Olivier Knox reports.

"I remain hopeful that productive conversations can be had in the days ahead, but the White House has to get serious," Boehner said. "This is not a game. Jobs are on the line. The American economy is on the line. And this is a moment for adult leadership."

The fiscal cliff is an automatic one-two punch of higher taxes scheduled to kick in Jan. 1 and deep spending cuts set to start the following day. Many economists say the economic recovery will be in jeopardy if a deal is not reached.

The fiscal cliff is certain to be the primary topic on the Sunday talk shows. One question likely to be addressed: Was the spirit of compromise that seemed present in Washington right after the election never genuine, and, even if it was, has it already disappeared?

Also worth noting this weekend: Saturday is World AIDS Day, and a Presidential Proclamation urges governors, U.S. territorial officials, and the American people to join in appropriate activities to remember men, women, and children who have lost their lives to AIDS and to provide support and comfort to those living with the disease.

And then there is this: President Obama and first lady Michelle Obama are expected to attend the Kennedy Center Honors Gala on Sunday. This year's honorees are blues musician Buddy Guy, actor and director Dustin Hoffman, comedian and TV host David Letterman, ballerina Natalia Makarova, and rock band Led Zeppelin, with individual members John Paul Jones, Jimmy Page and Robert Plant each receiving the award.

And this: Senate Majority Leader Harry Reid turns 73 on Sunday.

VATICAN CITY (Reuters) - And so it came to pass that in the eighth year of Pope Benedict's reign, some tabloid and social media decreed that he had cancelled Christmas.

The day after Benedict's latest book "The Infancy Narratives - Jesus of Nazareth" - was published on November 20, Vatican officials found some headlines they were not expecting.

"Killjoy pope crushes Christmas nativity traditions," read one tabloid headline, claiming that Benedict had snubbed traditions such as animals in nativity scenes and carolling.

"Pope sets out to debunk Christmas myths," ran another.

Holy Scrooge! Some blogs unceremoniously branded Benedict the new Grinch that stole Christmas and one rocketed him to the "top of the grumpy list for 2012."

And then there was this zinger headline from a web news site: "Pope bans Christmas".

Coming little more than a month before Christmas, it was the last thing the Vatican needed - another image problem for the pope.

Alarmed by some of the headlines, the Catholic social network XT3 felt compelled to run a blog that dissected the media's coverage of the book.

It was headlined: "The pope has not banned Christmas".

So what was all the fuss about?

In the 137-page book, the pope states a fact: that in the gospels there is "no reference" to the presence of animals in the stable - actually, it was probably a cave - where Jesus was born.

Bloggers had a feast with that, with one calling it "Bombshell number one".

What some neglected was that just a few sentences down, the pope states that even today, "No representation of the crib is complete without the ox and the ass".

He explains: The tradition of the ass and ox came from reflecting on parts of the Old and New Testaments. Christian iconography then adopted the motif early in Church history to show that even animals knew Jesus was the son of God.

KEEP ON CAROLLING

In other words, the tradition that has developed over the centuries matters more than an unverifiable fact, at least as far as the case of the ox and ass in the stable is concerned.

"I think that what people need to realise here is that the pope is trying to be as historical as he can be," said Father Robert Dodaro, professor of patristics, or the study of early Church writings, at Rome's Patristic Institute.

"He wants to see the biblical narratives as history where possible but he is also trying to explain details in the narratives that cannot be historically verified," he said.

Some bloggers, taking their cue from television and website headlines, even wrote that the pope had spoken out against Christmas carols.

In the book, the opposite was true.

Benedict says the evangelist Luke wrote that at the moment of Jesus' birth the angels "said" the well-known phrase "Glory to God in the highest, and on earth peace among men with whom he is pleased".

But in the next line he explains that "Christianity has always understood that the speech of angels is actually song", that "the angels' song of praise has never gone silent", and that it is "only natural that simple believers (even today) join in their carolling on the Holy Night".

So, no need to cancel any school performances.

Another section of the book that irked some bloggers is where the pope restates what biblical scholars have known for decades, if not centuries - that Jesus was born several years earlier than the first century AD.

Benedict writes that since King Herod died in 4 BC, Jesus was probably born "a few years earlier". He attributes the erroneous fixing of the year of Jesus' birth to a miscalculation by the monk Dionysius Exiguous some 500 years later.

"No one's faith should be shaken by this book," said Dodaro. "On the contrary, it should be fortified by this account. This is a believable account of the birth of Christ," he said.

And in St Peter's Square, workmen have started building the Vatican's larger than life nativity scene, which is expected to have animals and singing angels.

RELIGION | Contraceptive mandate challengers get first circuit court-level victory

The first ruling from a circuit court on the healthcare law?s contraceptive mandate went against the Obama administration on Wednesday. The 8th U.S. Circuit Court of Appeals issued a temporary injunction against the mandate on behalf of O?Brien Industrial Holdings, a Catholic-run mining company in St. Louis. On its website the business says its mission is ?to make our labor a pleasing offering to the Lord.? The Catholic owner, Frank O?Brien, objected to covering contraceptives for his 87 employees.

A federal judge dismissed O?Brien?s lawsuit in October, saying that because the owner was only indirectly paying for contraceptives, the mandate was not a burden on his religious freedom. But now the Catholic company is exempt from the mandate while the circuit court considers the merits of its case.

Courts typically issue a temporary injunction when the judges believe the challenge has some likelihood of success. At the District Court level, judges explained their injunctions against the mandate over many pages, but the 8th Circuit simply issued a two-sentence order saying that the injunction was granted. One judge on the three-judge panel dissented.

Four courts have now granted temporary injunctions against the mandate on behalf of religious business owners. In July, a U.S. District Court in Colorado ordered a preliminary injunction against the mandate for Hercules Industries, a Catholic-owned business. At the end of October, a U.S. District Court in Michigan ordered a preliminary injunction against the mandate for Weingartz Supply Company, another Catholic-owned business. And two weeks ago, a U.S. District Court in the District of Columbia granted a preliminary injunction to Tyndale House Publishers, a for-profit company that publishes Christian material (see ?Tyndale House wins mandate court battle,? by Mickey McLean, Nov. 17).

Religious colleges and nonprofit organizations have been less successful, as courts say they can?t consider such lawsuits until the mandate for religious organizations is in effect. Just last week a federal judge dismissed a lawsuit from a group of Catholic nonprofit groups in Tennessee on the grounds that they aren?t under the mandate yet. And on Tuesday, a federal judge dismissed a suit filed by the Catholic Diocese of Pittsburgh on the same grounds. But the mandate already applies to for-profit employers, religious or not, with more than 50 employees.

One federal judge did reject a request for a temporary injunction on behalf of evangelical-owned Hobby Lobby (see ?Not religious enough,? Nov. 20). The craft retailer has filed a request for an emergency injunction with the 10th U.S. Circuit Court of Appeals and hopes to hear an answer soon because it will begin facing fines of up to $1.3 million a day for not providing coverage for abortifacients to its 13,500 employees.

Appellate courts dealing with these religious business owners? cases are likely to issue a number of decisions before year?s end, because many businesses will start facing fines over the contraceptive mandate at the beginning of 2013 when their health insurance policies renew.

Mayim Bialik files to end 9-year marriage in LA

LOS ANGELES (AP) ? Court records show Mayim Bialik filed for divorce from her husband of nine years on the same day she announced the couple's split in a blog post.

She cited irreconcilable differences with husband Michael Stone in the documents filed Nov. 21 in Los Angeles.

Bialik currently stars on the CBS comedy "The Big Bang Theory" and rose to fame as the star of the TV show "Blossom."

She has been a proponent of "attachment parenting" and the former couple have two sons together, ages 7 and 4. Bialik has said their parenting style was not a factor in the divorce and she is seeking joint custody of the children.

The 36-year-old wrote in her post last week that the divorce is "terribly sad, painful and incomprehensible" for children.

(Credit: Dan Farber/CNET) NEW YORK--Facebook is the social network of the past, and the way it implements advertising doesn't really work, a Google executive overseeing the search giant's social-networking product said today.

Bradley Horowitz, the Google vice president of product for Google+, said Facebook isn't set up in a way that's compatible with the real world. People should be able to have a conversation with a certain group of friends without involving others. And they should be able to have real conversations, he said, not 140-character blurbs like Twitter.

"In designing Google+, we keep thinking about the real world, the way people actually are," Horowitz said today during a Business Insider conference in New York. "We're trying to make a product that's ergonomic for the way our attention is wired."

Meanwhile, Horowitz compared the ads in a Facebook user's newsfeed to a sandwich board. Such ads don't really have any context and often aren't very effective, he said.

"Jamming ads and agendas into user streams is pissing off users and frustrating brands too," he said. "That's not the way the world works."

Rather, in the real world, there has to be intent. When a person's hungry, he or she goes into a restaurant. Seeing an ad for a sandwich when they're not hungry or looking for it isn't very effective. But being able to search for a lunch place when hungry and finding recommendations from friends is much more effective.

"It turns out these are very valuable to users to have recommendations by the people they trust," he said. "Instead of sandwich boards... we revert back to the fundamentals of fulling the need the user has."

Horowitz added that Google doesn't "have to make payroll by jamming users with ads" on Google+.

(Credit: Shara Tibken/CNET) As Nicholas Carlson, the Business Insider editor interviewing Horowitz pointed out, Horowitz was essentially making an argument for Facebook to expand into search.

Search has been a pretty hot topic for Facebook in the past few months. Chief Executive Officer Mark Zuckerberg has said that Facebook is "pretty uniquely positioned to answer a lot of questions people have." It's sort of "friend mining" -- as CNET pointed out recently -- extracting specific answers to a question by mining the immensely data-rich social graph.

Horowitz, however, said today that "it turns out to be pretty hard" to make a search engine.

We've reached out to Facebook and will update when we hear back.

When asked if Google+ will ever incorporate ads, Horowitz said it would do so if there's an effective way to add them without upsetting users.

"We aren't struggling with how to monetize," he said. "We have real plans."

Google has tried social networking in the past but found little success. Its most recent attempt is Google+, which was launched last year to help it better compete with Facebook and other social networking sites. Google said in September that 100 million people are using Google+ each month, and it said it now has 400 million total users. That's still well below Facebook's user base but is a significant milestone for the new social network.

Horowitz declined to provide updated numbers today but said the figures from September are "stale." He added that Google+ is succeeding where Google's other social networking attempts failed largely because of the support of CEO Larry Page and other executives at Google.

"Larry has done something amazing," he said.

Google+ is designed around "Circles" that allow users to group people within their social sphere into different categories. It also includes a video chat feature dubbed "Hangouts," as well as other features.

Horowitz noted that Google+ currently is "foundational layer" for Google's other products -- the "identity, relationship, and interest system for Google." What it wants to be, he said, is the go-to place for people go to wish their friends happy birthday, much like Facebook is today except "uncluttered."

"We aspire to be a place where people can have uncluttered, meaningful connections. Communication is important. Like in the real world, context is important.... It's never fun to be late to a market, but it does afford us the opportunity to talk to users to see what needs aren't being met, what they like and don't like," Horowitz said.

Soldiers: Mexico beauty queen had gun in her hands

A Mexican beauty queen killed over the weekend in a shootout between suspected drug traffickers and soldiers likely was being used as a human shield, a federal official said Tuesday.

Maria Susana Flores Gamez, crowned 2012 Woman of Sinaloa in February, came out of the car first with a gun in her hands during the confrontation, with the other gunmen hiding behind her, according to the official from the attorney general's office.

He spoke on condition of anonymity because of the ongoing investigation.

The official said he read the military report of Saturday's shootout in Flores Gamez's hometown of Guamuchil in western Sinaloa state, home to Mexico's most powerful cartel of the same name. The attorney general's investigators are still trying to determine if the 20-year-old fired the gun she was holding.

The report said she went down in a hail of gunfire. She was found dead near an assault rifle along with two others.

"They used the woman as a human shield," the official said.

The slender, 5-foot-7-inch brunette had competed with seven other contestants for the more prestigious state beauty contest, Miss Sinaloa, but didn't win. Miss Sinaloa state winners compete for the Miss Mexico title, whose holder represents the country in the international Miss Universe pageant.

Mexico's Ximena Navarrete was crowned Miss Universe in 2010.

Local media outlets continue to misidentify Flores Gamez on Tuesday as Miss Sinaloa.

The organizers of the Miss Sinaloa pageant issued a statement on the pageant's Facebook page, seeking to make clear Flores Gamez was not their queen.

The misidentification "damages the image and tranquility of our queens, their families and friends," the statement said.

Neither the state nor national pageants responded to requests for comment on Flores Gamez's death.

It was at least the fourth documented case of a beauty queen or pageant contestant becoming involved with Mexican drug traffickers, the theme of the critically acclaimed 2011 movie "Miss Bala," or "Miss Bullet," Mexico's official submission to the Best Foreign Language Film category of Academy Awards.

The film tells the story of a young woman competing for Miss Baja California who becomes an unwilling participant in a drug-running ring, finally getting arrested for deeds she was forced into performing.

In real life, top Sinaloa cartel drug lord Joaquin "El Chapo" Guzman married local beauty queen Emma Coronel, who later crossed into the United States to give birth to twin girls in 2011.

In 2008, former Miss Sinaloa Laura Zuniga was stripped of her crown in the Hispanoamerican Queen pageant after she was detained that year on suspicion of drug and weapons violations. She was later released without charges.

In 2011, a Colombian former model and pageant contestant was detained along with Jose Jorge Balderas, an accused drug trafficker and suspect in the 2010 bar shooting of Salvador Cabanas, a former star for Paraguay's national football team and Mexico's Club America. She was also later released.

"A lot of young women are attracted by the false riches of the drug gangs. They offer the fantasy of a life of riches without much work," said Judith del Rincon, a women's rights activist and former Sinaloa legislator. "A lot of beauty queens wind up as girlfriends of some narco."

Del Rincon added that the involvement of drug lords with beauty queens dates back at least to the heyday of the Tijuana-based Arellano Felix drug gang in the 1990s.

Sinaloa state prosecutor Marco Antonio Higuera said Flores Gamez was traveling in one of several vehicles that engaged soldiers in an hours-long chase and gun battle. He said two other members of the drug gang were detained.

The shootout began when the gunmen opened fire on a Mexican army patrol. Soldiers gave chase and cornered the gang at a safe house in the town of Mocorito. Some men escaped, and the gun battle continued along a nearby roadway, where the gang's vehicles were eventually stopped. Six vehicles, drugs and weapons were seized following the confrontation.

Higuera said Flores Gamez's body has been turned over to relatives for burial.

Studies examine whether therapies for heart failure are associated with improved survival

ScienceDaily (Nov. 27, 2012) ? An analysis of two heart failure therapies finds differing outcomes regarding improvement in survival, according to two studies appearing in the November 28 issue of JAMA.

In one study, Adrian F. Hernandez, M.D., M.H.S., of the Duke Clinical Research Institute, Durham, N.C., and colleagues examined the clinical effectiveness of aldosterone antagonist therapy and associations with long-term outcomes of older patients discharged from a hospitalization for heart failure.

"Aldosterone antagonist therapy [a diuretic drug] for heart failure and reduced ejection fraction [a measure of how well the left ventricle of the heart pumps with each contraction] has been highly efficacious in randomized trials. However, questions remain regarding the effectiveness and safety of the therapy in clinical practice," according to background information in the article.

The researchers examined outcomes of eligible patients hospitalized with heart failure and reduced ejection fraction using clinical registry data linked to Medicare claims from 2005 through 2010. The primary outcomes measured for the study were all-cause mortality, cardiovascular readmission, and heart failure readmission at 3 years, and readmission associated with hyperkalemia (higher than normal levels of potassium in the circulating blood) at 30 days and 1 year.

Of the 5,887 patients (average age, 78 years) who met the inclusion criteria from 246 hospitals, 1,070 (18.2 percent) received a prescription for an aldosterone antagonist at hospital discharge. The researchers found that rates of all-cause mortality (49.9 percent vs. 51.2 percent) and cardiovascular readmission (63.8 percent vs. 63.9 percent) were similar between the treatment groups at 3 years. The cumulative incidence rates of arrhythmia (5.4 percent vs. 3.9 percent) and elective readmission for an arrhythmia control device (6.5 percent vs. 4.2 percent) were higher for the treated group. Analysis of the data indicated that there were no significant differences in mortality and cardiovascular readmission.

The authors also found that the cumulative incidence of the first heart failure readmission was significantly lower in the treated group (38.7 percent vs. 44.9 percent). The hyperkalemia readmission rates at 30 days (2.9 percent vs. 1.2 percent) and 1 year (8.9 percent vs. 6.3 percent) were higher in the treated group; however, hyperkalemia was seldom the primary diagnosis for these readmissions, and the absolute increase in hyperkalemia as a primary diagnosis was small.

The authors add that a potential explanation for their findings is that aldosterone antagonists have limited effectiveness regarding mortality in real-world settings among older patients. "One potential reason for limited effectiveness may be a lack of adherence to or persistence with therapy. ? Our findings highlight the importance of conducting clinical trials that can be easily generalized to real-world practice and in which the most vulnerable patient groups are well represented."

Use of ACE Inhibitors, ARB's, Associated With Improved Survival Among Certain Patients With Heart Failure

In another study, Lars H. Lund, M.D., Ph.D., of the Karolinska Institutet, Stockholm, Sweden, and colleagues conducted a study to examine whether renin-angiotensin system (RAS) antagonists (i.e., angiotensin-converting enzyme [ACE] inhibitors or angiotensin receptor blockers [ARBs] are associated with reduced mortality in heart failure patients with preserved ejection fraction.

"Up to half of patients with heart failure have normal or near-normal ejection fraction, termed heart failure with preserved ejection fraction (HFPEF) or diastolic heart failure. The mortality in HFPEF may be as high as in heart failure with reduced ejection fraction (HFREF) or systolic heart failure, but there is no proven therapy," according to background information in the article.

The study included 41,791 patients in the Swedish Heart Failure Registry, from 64 hospitals and 84 outpatient clinics between 2000 and 2011. Of these, 16,216 patients with HFPEF (ejection fraction of 40 percent or greater; average age, 75 years; 46 percent women) were either treated (n = 12,543) or not treated (n = 3,673) with RAS antagonists. Analyses was conducted of the data to determine the association between use of RAS antagonists and all-cause mortality, with use of a matched cohort. The researchers included 20,111 patients with ejection fraction of less than 40 percent for the HFREF consistency analysis.

In the overall HFPEF group, crude 1-year survival was 86 percent for patients receiving RAS antagonists vs. 69 percent for patients not receiving RAS antagonists; and 5-year survival was 55 percent vs. 32 percent, respectively. In the matched HFPEF cohort, 1-year survival was 77 percent for treated patients vs. 72 percent for untreated patients. Five-year survival was 36 percent vs. 34 percent, respectively.

"There is currently no consensus on the use of RAS antagonists in patients with HFPEF. In our study, use of RAS antagonists was associated with reduced all-cause mortality in a broad unselected population of patients with HFPEF. Our results together with the signal toward benefit in randomized controlled trials suggest that RAS antagonists may be beneficial in patients with HFPEF, but this should be confirmed in an appropriately powered randomized trial," the authors conclude.

Editorial: Heart Failure Therapy -- What Should Clinicians Believe?

In an accompanying editorial, James C. Fang, M.D., of University Hospitals Case Medical Center, Cleveland, examines the question of what physicians should conclude from these 2 observational studies that would appear to contradict the clinical trial evidence.

"If all of the evidence is carefully considered in its totality, it would be sound to conclude that (1) renin-angiotensin system antagonists are reasonable agents to control hypertension in heart failure with preserved ejection fraction, and (2) aldosterone antagonists are effective drugs in heart failure with reduced ejection fraction but should be used carefully and selectively. Although clinical trials should remain the gold standard for testing hypotheses, observational studies bridge the gap from the scientific rigor of clinical trials to real-world experience. Clinical trials are a reminder of the rigor of medicine as a science; observational studies are a reminder that medicine is still an art."

Genetic and chemical analyses of human skeletal remains reveal origins and food habits of first Sicilians

Skeletal remains in an island cave in Favignana, Italy, reveal that modern humans first settled in Sicily around the time of the last ice age and despite living on Mediterranean islands, ate little seafood. The research is published November 28 in the open access journal PLOS ONE by Marcello Mannino and colleagues from the Max Planck Institute for Evolutionary Anthropology, Germany.

Genetic analysis of the bones discovered in caves on the Egadi islands provides some of the first mitochondrial DNA data available for early humans from the Mediterranean region, a crucial piece of evidence in ancestry analysis. This analysis reveals the time when modern humans reached these islands. Mannino says, "The definitive peopling of Sicily by modern humans only occurred at the peak of the last ice age, around 19,000 -26,500 years ago, when sea levels were low enough to expose a land bridge between the island and the Italian peninsula".

The authors also analyzed the chemical composition of the human remains and found that these early settlers retained their hunter-gatherer lifestyles, relying on terrestrial animals rather than marine sources for meat. According to the study, despite living on islands during a time when sea level rise was rapid enough to change within a single human lifetime, these early settlers appear to have made little use of the marine resources available to them. The authors conclude, "These findings have crucial implications for studies of the role of seafood in the diet of Mediterranean hunter-gatherers."
